What You Need For Your Visit

• Medications:
  Please bring all of your medications from home.
• Identification:
  Your Insurance Identification (I.D.) Card. You are required to present this at each visit. This includes private insurance and Medicare/ Medicaid eligibility cards.
• Picture ID (Patient or Guarantor):
  You are required to present this at each visit. Acceptable forms include: valid drivers license, valid state id, military id or valid passport from country of origin.
• Medicare Prescription Drug Plan:
  Please bring your Prescription Drug Plan with you if you have Medicare.
• Handicapped Assistance:
  Please tell us if you need additional assistance. Call ahead if you wish.
• Laboratory and X-Ray Test Results:
  Lab results vary in result reading time and may not be released until reviewed by your doctor.
